Generic UPCC Referral Criteria

Inclusion Criteria

• Age 17 – 70

• Vital signs within normal range (NEWS2 ≤ 4, no single score of 3)

• Not intoxicated by drugs or alcohol

• No acute mental health concerns

• Cognitively intact

• Able to ambulate safely & independently (inclusive of those with mobility aides)

• Patient can tolerate oral rehydration

• Willing to redirect to UPCC

• UPCC can accept intake

Exclusion Criteria

• Serious trauma or dangerous mechanism of injury

• Cardiac symptoms

• Respiratory distress

• Abnormal vital signs (NEWS2 > 4, single score of 3)

• SIRS or possible sepsis

• Suspected stroke or TIA

• Recent seizure activity

• Acute mental health concern

• Acute substance use concern (withdrawal)

• Infectious disease requiring isolation

• Patients in Custody
